You've registered with the BPS and started paying FONASA. The next step is choosing your health provider — your mutualista. The natural question is: which is the best? Here's the honest answer, plus how to compare them with official data and, step by step, how to affiliate.
Which mutualista is best? The best one for you
There's no single “best” mutualista for everyone: it depends on where you live, how quickly you get an appointment, how much they charge in tickets and órdenes, and how satisfied their members are. The good news is you don't have to guess: the MSP publishes an official site, ATuServicio, where you can compare every mutualista with real data.

When you open ATuServicio (or when you ask around), pay special attention to these points:
- Waiting times: how many days until the first available doctor, by specialty.
- Ticket and orden prices: what you'll pay day to day for consultations, tests and medicines.
- User satisfaction: the MSP survey on members' real experience.
- Proximity and facilities: where their clinics and hospital are relative to your home and work.
- Resources and specialties: availability of staff and of the specialties you use most.

How to affiliate, step by step
Once you're registered with the BPS and generating FONASA, affiliating is simple. The path is:
- Confirm you're registered with the BPS and that your activity gives FONASA rights.
- Compare the options on ATuServicio by waiting times, prices and satisfaction.
- Choose within 30 days of starting. If you don't choose in time, you're assigned to ASSE.
- Go in person to the chosen mutualista with your valid cédula de identidad (ID).
- Coverage starts on the first day of the next month; until then you stay covered where you were.
- If your data is up to date, the BPS confirms the affiliation by SMS or email.
The procedure is in person: to affiliate (or switch) you have to go yourself to the chosen mutualista, with your ID.

In English: The right to care at the new health provider will take effect from the first day of the month following the change (keeping the right at the previous one until then).
